APPLICATION NARRATIVE

1. Describe the purpose or mission and give a brief history of the organization.

2. Briefly describe the project; specifically address how the project meets the criteria for funding (described in the program information).

3. Who will be served by this project and how will they benefit? How many
artists and/or students will be participating? What is the anticipated number of
public served?

4. Who is the project director? How is s/he affiliated with the applicant
organization? What will his/her function be? Is s/he employed by the
organization full-time or part-time, or a volunteer?

5. How will you make accommodations for handicapped accessibility?

6. What is your organization's previous experience in carrying out similar
projects?  Give a specific example.

7. What sources of funding (both in-kind and cash) will you have for this project
other than funds from this program?

8. Describe how this program will be evaluated or assessed.

9. What assurance can you provide that the project will occur?

  • A copy of your organization's most current budget

  • A copy of letter certifying applicant organization's 501(c)(3) tax-exempt status

  • Resume of staff member(s) or volunteer(s) responsible for the project

  • Name and affiliation of board members, or committee, responsible for making decisions about how funds will be expended
     
  • Support letters from individuals or groups expected to benefit from the project.
     
  • Any additional information that would assist the panel in its review.
